Synthetic (Biofiber) Hair Transplant: How It’s Done, Pros and Cons

Synthetic hair transplantation — also called biofiber — implants man-made fibres, available in different colours and lengths, as an alternative to your own follicles. It is mainly considered by people with complete baldness whose donor area cannot supply grafts for a natural transplant. Results are immediate, but there are real trade-offs to understand.

What is a synthetic hair transplant?

A natural transplant moves follicles from a donor area to the bald area — but many people with complete baldness lack a usable donor area. Biofiber offers an alternative: synthetic fibres, biocompatible and made to resemble natural hair, are implanted directly into the scalp.

A brief history

Synthetic hair began in the early 1970s. In 1983 the US FDA banned biofiber products because of poor biocompatibility and immune reactions as the body rejected them. In 1996 an Italian company introduced a more biocompatible biofiber, later approved in the European Union — though it is used sparingly, since some patients need the procedure repeated.

Requirements before the procedure

• The patient’s suitability must be assessed with several medical tests beforehand.
• The body’s reaction to the synthetic hair is monitored weekly for at least a month — a small number of test implants are placed before larger sessions, and the procedure only proceeds if the patient passes these tolerance tests.

How it is done

• Polyamide-fibre hair strands are used.
• The procedure is done under local anaesthetic with an implanter pen.
• The device inserts each strand to a depth of about 0.3 mm, similar in principle to the DHI technique.
• Micro-channels are opened one by one with the implanter.
• Each fibre is anchored by a knot placed beneath the scalp; as the body closes the channels with connective tissue, it locks the knot in place.

Benefits

• Immediate new hair — the result is visible right after the procedure.
• Natural appearance, as fibres are placed individually and can be styled.
• No incisions and no need for a donor area.
• Preserves the density of hair surrounding the treated area.
• Lower cost than a natural transplant, and no recovery downtime.

Drawbacks to weigh carefully

• The body may reject the fibres and react with itching or inflammation.
• Temporary swelling for a few days is possible.
• Minor scalp bleeding can occur for a few hours.
• Around 15–20% of the implanted fibres are shed each year, so top-ups are needed annually.
• The lifespan of the fibres is roughly eight to ten years, depending on the body’s response.
• If the body later rejects the fibres, they must be removed.
• A skin infection at the site can prevent a future natural transplant in that area.

Aftercare tips

• Handle the synthetic hair gently and naturally.
• Use a wide-tooth comb, combing in the direction of the implanted hair.
• Wash regularly with a gentle, careful technique.
• Rinse with fresh water after swimming in the sea or a pool.
• Protect the hair with a damp towel when exposed to heat, such as in a sauna.
