Hair Transplant Problems Before and After Surgery, and 6 Contraindications

Understanding potential hair transplant problems — and who the procedure isn’t suited for — is the key to a result you’ll be happy with. Here are the factors to weigh before surgery, the normal side effects afterwards, and how to avoid them turning into complications.

Problems and contraindications before surgery

1. Wanting a transplant to add density where hair is already plentiful, which can leave an uneven result.
2. Not having enough donor follicles — from the scalp or body — to work with.
3. Needing to wait until genetic loss stabilises; many surgeons prefer to wait until about 25 (and not below 20) so stable donor follicles can be identified.
4. Scalp conditions that rule the procedure out — chronic or autoimmune disease such as alopecia, psoriasis or eczema, or thick scarring.
5. Difficulty selecting a stable donor area in early or ongoing loss, which can weaken the final result if more hair sheds later.
6. In women, a lower proportion of good candidates — hormonal or genetic loss appears as scattered thinning, making the donor area harder to define.

Problems and side effects after surgery

Some side effects are completely normal and only become “problems” if care instructions are ignored:

1. Swelling of the forehead and scalp is normal and settles within weeks — sleep on your back, avoid salt, eat well and drink at least 2 litres of water a day.
2. Small pimples can appear as new hair grows; painful or itchy ones may signal infection or poor hygiene, so check with your doctor first.
3. Itching is normal as follicles and tissue heal; use the prescribed medication and never scratch the transplanted areas.
4. Scabs form as follicles heal — keep them moisturised and clean and they fall away on their own; picking them can damage or move the follicles.
5. Shedding of the transplanted hair around week three is completely normal and makes way for permanent new growth.

Results

Results show from the third month as shedding stops and new hair grows; by the sixth month most of the area is covered, and the final result appears at one to one-and-a-half years, when your surgeon can advise whether a small touch-up session is needed.
