What is gynecomastia?
Gynecomastia is enlargement of the male breast, where one or both are larger or more prominent than normal. It’s a sign rather than a disease, and it affects men at different ages for different reasons.
Hormonal causes
The main driver is an imbalance between testosterone (the male hormone) and oestrogen (the female hormone) — when oestrogen rises or testosterone falls, breast tissue can grow. Triggers include puberty, ageing, endocrine disorders, certain medications (some heart, anti-anxiety, ulcer and prostate drugs, and anabolic steroids), obesity, and liver or kidney disease.
Diagnosis matters
Early, accurate diagnosis guides the right treatment. Your doctor takes a clinical exam and history, and may order hormone blood tests, an ultrasound or mammogram, and — if needed — further imaging, to confirm whether the enlargement is glandular tissue or fat and to rule out other causes.
Medical (drug) treatment
When gynecomastia is caused by a hormonal imbalance, medication can help: options include testosterone support where it’s low, and oestrogen-blocking drugs such as tamoxifen, aromatase inhibitors, danazol or clomiphene. Drug treatment works best in earlier, hormone-driven cases and is decided by your doctor.
